Provides direct and indirect care in meeting the general health and personal care needs of program members in a safe, efficient manner. Performs personal cares. Coordinates and maintains the integrity of all health records and information pertaining to member’s personal care needs.  Performs other duties as required.



  1. Primary personal care provider responsible for the dispensing of prescribed medications to members during program hours.
  2. Monitors general physical and psycho social care needs in collaboration with Case Manager and R.N.
  3. Varied direct care duties may include but not limited to: checking blood sugars, personal cares (including toileting), assist with set-up and direct feeding and ambulation assistance.
  4. Serves as a first responder in a medical emergency under the supervision of R.N.
  5. Organizes and maintains the integrity of all health records and information pertaining to members personal care needs.
  6. Develops and maintains the personal care schedule and personal care plans in collaboration with the case management team.
  7. Assists the Case Manager in the implementation of participant’s person-centered plans, which includes making behavioral observations, assisting with situational assessments, and facilitating attainment of participant’s goals.




Work performed is generally within the medium range as defined by the United States Department of Labor.



  1. CNA coursework and certification.
  2. First aid and CPR certified.
  3. Certified to dispense medications preferred.
  4. Demonstrated common sense, judgment, communication skills and the ability to maintain confidentiality.
