Appleton Day Program Offers Person-Centered Planning

Based in Appleton, Wisconsin, the Day Services Program at Valley Packaging Industries - Brewster Street is a multifaceted program serving adults who have moderate to profound impairments and are challenged by multiple disability characteristics. It provides person-centered planning and alternative day programming for individuals whose capabilities suggest success in areas other than participation in community and/or facility-based work activity.

Programming includes regularly scheduled, individualized skill enhancement activities and goals may address the development/enhancement of a participant’s skills in these areas:

  • Social integration
  • Communication
  • Motor and eye/hand coordination
  • Wellness

With this day program, community integration activities may also be offered in a self-paced environment consistent with defined needs, abilities and preferences.

The Day Services Program is divided into two distinct groups:

  • Level I: The target population for this level is individuals who are elderly and frail. A specific criterion for participation in this level of service is that individuals do not demonstrate disruptive combative behaviors.
  • Level II: The target population for this level is individuals who have moderate to profound cognitive impairments. Participants may exhibit multiple disability characteristics, lack of self direction and present challenging behavioral needs which require extensive staff support to be successful.
