COMMUNITY 2000

A Community Outreach Program for residents of the Seymour & Shiocton School Districts

Community 2000 is all about helping people. While many individuals do not qualify for county assistance because their income is outside of set parameters, they still find it difficult to survive on their limited earnings. Community 2000 can provide assistance along with information and referrals for those who may not realize that other options exist.

Community 2000 provides services to individuals and/or families residing in the Seymour and Shiocton School Districts who have a gross monthly household income at or below 200% of the poverty level. These community outreach services include:

Emergency Services

  • Emergency financial assistance with rent or utilities
  • Local contacts for other rural community agencies
  • Emergency food pantry services
  • Local appointments for county wide agencies at the rural facility:
  •   • Energy Services (heating assistance)
            • LEAVEN
            • Harbor House
            • CAP Services
            • Aging & Disability Resource Center (ADRC-disability/medicare/Medicaid)
            • FISC (create a household budget at no charge)

Special Projects

  • Back to school supplies for children and adults
  • Christmas assistance for the entire family along with a special meal and pantry items
  • "Kids Day" for children to meet Santa and pick out a gift for their caregivers at no charge
  • Easter baskets for the younger children

Material Goods

  • Emergency clothing needs
  • Bedding of various sizes
  • Miscellaneous household and kitchen items

Information and Referrals

  • Local food pantries
  • Outagamie County Health and Human Services
  • Referral information for surrounding counties
  • Referrals to other services within our county

Voucher Programs

  • Vouchers for the local farmers market
  • Free haircuts
  • Reduced cost oil changes
  • Vision exams
  • Reduced cost pharmacy card
  • Vouchers for basic needs for furniture, etc.
